SHARED CARE AGREEMENTS - IMPORTANT INFORMATION
- Prior to requesting a Shared Care Agreement letter from Dr Sadiq, please can you ensure that you have spoken to your GP about this and if possible, send confirmation from them in writing to pa-dr-sadiq@augmentive.io. Augmentive are aware that many GPs now have a policy to refuse Shared Care Agreements and we are unable to influence their decision. Our inclusion criteria as a Clinic states that our services "are for for patients for whom assessment and ongoing treatment is financially viable. Each patient would need to make an assessment of that themselves."
- Confirmation from your GP will be crucial to being able to initiate the Shared Care Agreement with them once you are titrated and stable on your medication. Dr Sadiq is not able to ask for the GP to take over prescribing for you until you are stable and have been for three months.
· Please see FAQs below for all queries regarding medication and prescriptions.
